2. Website access
When the website is accessed, the hosting provider processes technically necessary connection data such as IP address, time, requested page and browser or device information. This supports secure and reliable delivery. The site uses Cloudflare infrastructure, which may involve processing outside the European Economic Area.
3. Online booking
Booking data includes the selected treatment, date, time, first and last name, email address, telephone number and privacy confirmation. It is used to reserve the appointment, prevent double bookings and communicate about it. Please do not submit additional diagnoses or health details through the form.
Bookings are stored in a protected database provided by the host. Brevo is used for notifications and confirmations. Retention periods remain to be confirmed in the final document.
4. Contact
Information sent through the form or by email is processed to answer the enquiry. The form uses Brevo to deliver messages securely.
5. Google Maps link
No map is loaded automatically. Selecting the external link takes you to Google Maps, where that provider's terms apply.
6. Your rights
Subject to legal conditions, rights may include access, correction, deletion, restriction, portability, withdrawal of consent and objection. Contact info@om-skin.com. Complaints may also be made to the Polish data protection authority, the President of the Personal Data Protection Office (UODO).
7. Final version
The document will be completed and legally reviewed after the controller, address, processors and retention periods have been confirmed.
